Before training begins:

* Must have a medically substantiated physical or psychologic disability.

 

 * Submit a Medical Prescription when requested.

* Submit a completed application .

​

* Meet with At Your Service Dog Training either in person or via the internet to find out more about the program and costs.

​

* Complete a comprehensive evaluation by At Your Service Dog Training, LLC, of your dog, a dog you presently own or a new dog.

​

* Present proof the dog is current on all age appropriate vaccines.

​

* Complete the Service Dog Training Agreement.
